Home and Community-Based Services (HCBS)

HCBS is for adults with Medicaid or Polk County funding who have a diagnosis of a developmental disability, brain injury, chronic mental illness, or intellectual disability that want to live in the community and have a fully-inclusive, fun, and healthy life and need a variety of supports to do it.

Candeo passionately supports individuals to work toward being in the driver’s seat of their life and services. They pledge to educate people while letting them experience life like people without disabilities. At times, that means letting people make mistakes, and learn through the consequences of those decisions, without abandoning people to harmful choices.

Candeo advocates for the people they support. We do not give up on people because of setbacks or other struggles because often, the Candeo staff can see a person’s potential for success before they can.

Candeo provides these services, often called Supported Community Living Services, or Community Living Services, in people’s homes where they may get support for a few hours a day (hourly services) up to 24 hours a day with roommates (daily services).
