Home and Community-Based Services (HCBS)

Host homes are for adults with Medicaid or Polk County funding who have a diagnosis of a developmental disability, brain injury, chronic mental illness, or intellectual disability that want to live in the community and have a fully-inclusive, fun, and healthy life and need a variety of supports to do it.

Community-based living services are supports provided to people with disabilities who want to be a contributing member of their community. The host home model of support is where the client lives in the private home of an approved Independent Contractor who provides all of the needed supports. The client is a part of the independent contractor’s family and engages in all family events. 

Supports and coaching may include: cooking, cleaning, laundry, shopping, paying bills, taking medication, managing diabetes or other illnesses, going to medical appointments, calling up friends to go to a movie, hosting a pizza party, going to museums, Adventureland, Mall of America, flying home to visit mom and dad, etc.

Candeo advocates for the people they support. We do not give up on people because of setbacks or other struggles because often, the Candeo staff can see a person’s potential for success before they can.
