Home-Based Habilitation

Home-based habilitation is for adults with a diagnosis of a chronic mental illness, want to live in the community and have a fully inclusive, fun, and healthy life but need a variety of supports to do this. Those with Medicaid or Polk County funding already have resources for those supports to make community life a reality.


Candeo truly supports individuals to work toward being in the driver’s seat of their life and services. They pledge to educate people while letting them experience life like people without disabilities. From time to time, that means they let people make mistakes and learn through the consequences of those decisions, without abandoning people to harmful choices.


Candeo advocates for the people they support. We do not give up on people because of setbacks or other struggles because often, the Candeo staff can see a person's potential for success before they can.

 

Candeo provides a service called Home-Based Habilitation for people with disabilities who want to be contributing members of their community. This service is based in the home, with services ranging from 10 hours per month to 24-hour support and supervision. Our staff supports individuals to do everything from cooking and cleaning to calling up friends to go to the movies.
